How does one "insert coins" in this game? I can't start to play it because, according to instructions, it's the "insert" key, which is shared by 0 in my keyboard's numeric block, but it doesn't insert any coins :(

It is Insert - I have a similar problem on my keyboard, you should have a key that toggles between 0/Insert, in my case I have to hold "Fn"
